The numbers are almost hard to believe. Eli Lilly (LLY) posted Q1 2026 revenue of $19.80 billion — up 55.5% year over year — with non-GAAP EPS of $8.55 against a Wall Street consensus of $6.79. Volume across the business climbed 65%, even as realized prices fell 13% on rebates and access deals. Management responded by raising full-year revenue guidance to $82–$85 billion, implying another 27% growth year. That’s not pharma. That’s tech-tier compounding out of a drug portfolio.
And yet — here’s the part that gets interesting — the stock is trading at roughly 26x forward earnings, below its five-year mean of 34.56x. The consensus analyst price target sits at $1,224, implying ~32% upside from current levels. Guggenheim just raised their target to $1,235 from $1,183, maintaining a Buy.
The Two-Horse Race and Why It’s Splitting
Lilly now controls just over 60% of the U.S. GLP-1 market versus 39.4% for Novo Nordisk. Mounjaro alone generated $8.66 billion in the quarter — up 125% — fueled by international expansion including China’s National Reimbursed Drug List. Zepbound added $4.16 billion in U.S. revenue, up 80%. These are not incremental gains. This is category dominance compounding in real time.
Meanwhile, Novo Nordisk (NVO) is navigating a completely different reality. The company guided for 2026 sales to contract between 4% and 12% — its first sales decline since 2017. Pricing pressure in the U.S., semaglutide patent expirations in China, Brazil, and Canada, and Lilly’s prescription share gains are all hitting simultaneously. In Q1, Wegovy injectable sales grew just 12% year over year to 18.2 billion Danish kroner, missing expectations. Ozempic sales fell 8%.
